Emergency Card
For your child’s safety, an emergency card is required by all. The card must be signed by both the parent or guardian and student. You are able to fill out your child's emergency card online:

Immunizations
Parent/guardians of students who were informed of their student’s need to receive additional immunizations must have their student provide written physician/clinic documentation of specified immunizations by the deadline indicated.
Medication Card
The “Over the Counter Medication Record” card lists the noted medications that are provided by the Nurse’s Office and can be administered to your child upon completion/signature and the returned form to the Nurse’s Office. All other medications must be provided by the parent/guardian in the original container. A “School Year Record” form must also be completed and signed. Students are not allowed to carry any medication on their person, including Tylenol, Midol, Ibuprofen, etc.
Health Screening
Our annual September health screening requires approximately 30 volunteers to assist us with timely completion of the Arizona requirements. No experience required to sign up. If you are available to assist us please fill out the form below.

Influenza is spread easily from person to person; when someone who has it sneezes, coughs or even talks, the virus passes into the air and can be breathed in by anyone nearby. People can also become infected by touching something, such as a surface or object, with influenza virus on it and then touching their mouth or nose.
